This is a site that I have been a member of for nearly a year, but I stopped using it and have just revisited it today.
WHAT IT IS:
Cash Crate
HOW IT WORKS:
You sign up using a valid mailing address (they pay by check)
You complete as many of the "survey" offers as you wish. They usually have a payout ranging from $.20 to $7.00. (But the lower paying ones are easier to do and get verified much faster)
When you reach the minimum payout threshold of $20 they will mail you a check on the 15th of the following month.

I recommend creating a new email account or using one you don't use very much, because you will get a lot of spam email.
TIPS ON COMPLETING SURVEY OFFERS:
First of all, you should make sure you set the search filter to "100% free" and "Rating" Then click Go

Each "survey" has an explanation of what you need to do in order to earn the cash.
For example, it might say something like, "Register with valid information and complete the survey. Click atleast two offers on the "Final Steps" page."
You click on the link which will take to you the website.
I am going to warn you...These "surveys" are the most annoying kind you will come across and it is easy to get lost in a sea of pop up adds and unnecessary advertisements.
Most of these pop up sites will try to distract you and get you to enter your credit card information somehow.
DO NOT EVER ENTER YOUR CREDIT CARD OR SOCIAL SECURITY INFORMATION. AND BE CAREFUL ABOUT ENTERING CELL PHONE NUMBERS, BECAUSE SOMETIMES THERE IS FINE PRINT THAT SAYS YOU WILL BE CHARGED.
If something is asking for this type of information...ignore it and move on.
You will also notice that these "surveys" are never-ending.
But you don't actually have to get to the end of the survey to be rewarded the money on Cash Crate.
For an offer like this all you really need to do is enter your email address, shipping information, and then look through a bunch of advertisement offers (these offers are spam offers so it gets kind of repetitive).
Just click NO on all of the offers.
This is important because you don't want to waste your time on pointless spam deals. You can probably stop after a few pages.
When you are done, just go back to cashcrate and click submit next to the offer you completed and your money will be added to your pending funds. When the website verifies that you completed the survey, the money moves into the verified earnings.
It sounds complicated and it can get frustrating, but when you are used to it, you tend to use common sense and know how far you need to go in a survey to qualify for the reward on cash crate.
DOES IT REALLY PAY:
Yes!!! About a year ago when I first started using this site I earned $10.10 and I received my check in the mail. It was a nice surprise. I just started using this site again today, and I have earned $10.65 (in less than a day!) verified already and $13.25 still pending. I have a lot more offers to complete still too! Below is a screen shot of my earnings.

WHAT I LIKE ABOUT THIS SITE:
There are usually plenty of opportunities to complete offers and earn more money. Even though the rewards seem small, they really add up since there is not a limit to how many you can do in a day.

NEW UPDATE 03/03/09
I guess you might not have to go as far in the surveys as I thought. You might be fine if you just enter your email, shipping information, and then skip through a few pages that's it. I tried that and my offers seemed to confirm fine. Another thing I have noticed is that getting a new email every now and then seems to increase the number of offers that confirm for me. You can easily get a new gmail email address anytime. Just use that address when doing offers, your offers should confirm based on your computer's IP, so which email you use doesn't matter. Also, when you see that certain websites already have some of your information stored, you might want to click out of it, clear your privae data and cookies, and then start again. Someone told me this also helps offers confirm, and I guess it's true because I went back and redid the ones that had been pending for a while and it worked.
